Your First Visit
On your first visit you will be asked to arrive fifteen minutes early to
fill out several forms. Please remember to bring your insurance
information if you would like us to bill your insurance company. You
will be asked to bring your actual x-ray films with you to your visit.
These x-rays are a valuable tool to enable your doctor to complete your
evaluation. Your doctor will review your x-rays, perform a physical
examination, discuss the findings of this examination and a plan of
treatment. Your plan of treatment may include a needle biopsy which
will be performed at the time of your consultation or surgical
intervention which will be scheduled to be performed at a later date.
Family members are welcome
and encouraged to be with you. A cassette tape recording of the
consultation maybe made so that you and your family can review the
discussion later.
In order for your doctor to complete the most comprehensive consultation possible we would ask you to bring certain items.
If you are unable to make your appointment or will be late, please call as soon as possible.
Payment
for your consultation or consultation co-payment is due at the time
service is rendered. We accept cash, checks, Visa, or Mastercard. We
will be happy to process your insurance claim form for reimbursement.
We are a contracted provider for many insurance companies including
Medicare.
If your insurance company requires an
authorization, please bring with you. If not done, you maybe
responsible for your office visit.
Please print and fill out the 3 forms below. Bring them with you to your first scheduled visit. It is helpful to have these completed before your arrive at our office to be seen by the physician.
